Service Coordination Description
What services are offered?
Service coordination for persons enrolled in Home and Community-Based Services (HCBS) Waiver Program, Medicaid Service Coordination, the Early Intervention Program, or the Department of Health Waiver for individuals with traumatic brain injury.
Who is eligible for this service?
Individuals with developmental disabilities who are Medicaid eligible; individuals with traumatic brain injury who are Medicaid eligible; children age 0-3 with developmental delays.
